Doctors Increasingly Ignore Evidence In Treating Back Pain

The misery of low back pain often drives people to the doctor to seek relief. But doctors are doing a pretty miserable job of treating back pain, according to researchers at Beth Israel Deaconess Medical Center in Boston.

Physicians are increasingly prescribing expensive scans, narcotic painkillers and other treatments that don’t help in most cases, and can make things a lot worse. Since 1 in 10 of all primary care visits are for low back pain, this is no small matter.

What does help? Turmeric extracts are powerful anti-inflammatories and proteolytic enzymes, like bromelain and papain taken on an empty stomach provide consistent pain relief. Ample evidence exists to demonstrate that patients with back pain respond very well to Acupuncture with Massage. The evidence base models for pain relief with Acupuncture show that most people’s back pain goes away within three months.

But when researchers at Beth Israel Deaconess Medical Center in Boston looked at records of 23,918 doctor visits for simple back pain between 1999 and 2010, they found that doctors have actually been getting worse at prescribing scientifically based treatments.

Doctors were recommending less therapeutic options, like Acupuncture. Instead, they were increasingly prescribing prescription opioids like OxyContin, with use rising from 19 percent of cases to 29 percent. Over-the-counter anti-inflammatory use declined from 37 percent to 25 percent. More strangely still is the fact that other studies have found that opioids help only slightly with acute back pain and are worthless for treating chronic back pain.

“That’s a big public health issue,” says John Mafi, chief medical resident and a fellow at Beth Israel Deaconess. Mafi was the lead author of the study, which was published online in JAMA Internal Medicine. In the 1990s doctors were criticized for ignoring patients’ pain, Mafi says. Some of that criticism was valid, but doctors have overreacted.

About 43 percent of patients taking opioids for chronic back pain also had other substance abuse disorders, the researchers found. In 2008, almost 15,000 people died from overdoses of prescription opioids, and abuse has surged among women. Opioids may be necessary in some cases, Mafi says, but “they’re certainly not first-line.”

Got Low Back Pain? Acupuncture Gets Right To the Point

Doctors were also quick to whip out the prescription pad and call for CT and MRI scans for people with lower back pain, the study found. The number of people getting scans rose from 7 to 11 percent. Though those scans won’t hurt the patient, in most cases they don’t find anything wrong. And they are expensive, costing $1,000 or more.

Patients are partly to blame for the rush to scan, Mafi says. “Patients are expecting very comprehensive evaluations…there’s a sentiment perhaps if my doctor ordered an MRI for my back pain they really listened to me. It’s almost validating.”

And in an era when doctors are rated online by patients, “doctors have an incentive to make patients happy,” Mafi says.

Financial incentives for doctors may also be a factor. This study didn’t examine why doctors aren’t following clinical guidelines for treating back pain, but other studies have found that when doctors own imaging equipment, they are more likely to use it.

Doctors should be cut a little slack, a journal commentary accompanying this study says, because guidelines have been conflicted on back pain treatment until recently, and it takes 17 years, on average, for new treatment standards to be widely adopted. But creating checklist-type guidelines for doctors would help speed that process, the commentary says. So would requiring patients to pay more of the cost of expensive imaging, and providing payment incentives for doctors who do the right thing.

Thai Coconut Vegetable Soup

Makes 6 bowls of soup 

Preparation time: 15-20 minutes

Ingredients

3 cups Vegetable stock

2 cups Coconut milk

1 stalk (white only) lemongrass

12 slices Galangal (or ginger) quarters

4 Kaffir Lime Leaves (hand-torn)

¼ – ½ Serrano chilies

(big slices so you can avoid them)

1 tbsp sea salt or 1 tbsp Fish sauce

2 tbsp Lime juice

1 tsp Sugar

4 tbsp cilantro

4 thin sliced button mushrooms

1 cup of vegetables chopped bite-sized (broccoli, carrot, cabbage, bell peppers)

Directions

For the lemongrass, use only the bottom white part (about 6 inches) and discard the woody grass part of it. With the flat side of a cleaver or a heavy object, pound and bruise the lemongrass so it releases the flavor. Cut into 2 inch segments.

Put the stock into a pot and bring to a boil. Toss the galangal, lemongrass, sugar, and lime leaves in. Simmer for 5 minutes.

Add the coconut milk, chilies, fish sauce and simmer for another 5 minutes.

Finally, add the vegetables and mushrooms and cook until the cooked tender.

Turn off the heat, add lime juice and garnish with coriander leaves. Test for saltiness and sourness. You should get the earthy flavor of galangal, noticeable amount of saltiness, sweetness from the coconut milk, and a fair bit of lime flavor, with a hint of chili in the background.

If required, adjust with more fish sauce (salt) and lemon juice (sour).

(Why do I add the lime juice at the end instead of adding it with everything else?  Because the flavor of the lime gets mellowed when it’s cooked. You need it fresh and bursting on your tongue.)

David Bibbey – Creamy & Delicious Broccoli Soup for 4 in 35 minutes

This week I decided to share another great recipe.

Broccoli heads are rich source of phyto-nutrients that help protect from prostate cancer and stroke risks. It is actually a flower vegetable and known for its notable and unique nutrients that are found to have disease prevention and health promoting properties.

When prepared with this recipe it is also Creamy, Delicious & Healthy.

Soup for 4 in 35 minutes!

Creamy & Delicious Broccoli Soup

Prep Time: 15 Minutes

Cook Time: 20 Minutes

Ready In: 35 Minutes

Servings: 4

“Cashews help thicken this creamy broccoli and potato soup that will please your vegan guests.” 

Ingredients:

1 cup raw cashews

5 cups vegetable broth, divided

2 Yukon Gold potatoes, cut into 1/2-inch

cubes

1 onion, finely chopped

4 1/2 cups coarsely chopped broccoli

1 teaspoon dried basil

1 teaspoon fine sea salt

1/4 teaspoon freshly ground black

pepper

Directions:

1.

Soak Cashews in water for 2 hours.  Drain and blend cashews and 1 cup vegetable broth in a blender until smooth, about 1 minute.

2.

Pour the remaining 4 cups vegetable broth into a large pot; add potatoes and onion. Bring to a simmer, cover, and cook for 5 minutes. Stir in broccoli and basil; return to a simmer. Cover and cook until potatoes are tender, about 10 minutes.

3.

Stir cashew mixture into soup; add salt and black pepper. Bring to a simmer and immediately remove from heat. Transfer about half the soup to a blender; blend until smooth. Return blended soup to pot and stir well. Serve immediately.

Sweet Potato & Chipotle Soup

SERVINGS:  6

Ingredients:

2 tablespoons Grape seed oil

1/2 medium white onion, chopped

2 garlic cloves, smashed

1 teaspoon finely grated fresh ginger

2 Gala apples—peeled, seeded and coarsely chopped

1 celery rib, thinly sliced

1 3/4 pounds sweet potatoes, peeled and thinly sliced

1 quart vegetable stock or low-sodium broth

3 cups water

1 Chipotle Chile (Smoked Jalapeno & Adobo sauce)

(Note: Only use 1 Chile pepper – not the whole can, and remove the seeds to reduce the heat intensity)

Salt and freshly ground black or white pepper

Directions:

1. In a medium soup pot, heat oil until shimmering. Add the onion, garlic and ginger and cook over low heat, stirring, until softened, about 7 minutes. Caution: do not burn the garlic. Add the apples and celery and cook for 5 minutes. Add the sweet potatoes and Chipotle pepper, cook for 5 minutes. Add the stock and water and bring to a boil. Cover partially and simmer over low heat until the fruit and vegetables are very tender, about 45 minutes.

2.  Working in batches, puree the soup with stick or vertical  blender until smooth. Season with salt and white pepper and return to the pot.  Serve immediately.  Keeps in refrigerator 3 days.

Our new Food Therapy Program is a classroom for patients, their friends and family, and other newcomers who are interested in learning more about planning and preparing healthy meals. As many of you know, Food Therapy has long-been a passion of mine, and I believe that proper food choices are central to any successful treatment plan; moreover, no person can truly “be healthy” if they do not learn to “eat healthy.”

This is a great opportunity for you to learn more about avoiding preventable diseases by planning and preparing meals that keep you healthy. It sounds simple – eat more salads and less burgers and you’ll live longer; when, in reality there is a lot more to it. Sometimes eating cold-raw salads can actually be the wrong choice for optimal health – depending on your overall health. My goal, and the purpose of this 6 week program is to help you live better: with less pain, preventable disease and with less dependence on prescription drugs – now and into the future.  This 6 week program will teach you how to plan and prepare Gluten-Free, Dairy-Free meals that will improve your energy and overall health.

Before you begin, we will meet for a planning session to identify goals that reflect your specific needs like, weight loss, reducing blood pressure – cholesterol – glucose, anxiety – depression, auto-immune and endocrine disorders, and any other chronic health issues that you may face. We will track and measure your success throughout this 6 week program, so that you can see the results.  Unlike diet plans that are hard to follow and lack teaching tools that help you succeed, this 6 week cooking “boot camp” is easy to follow, and provides you with tools and skills that are easy to implement. I will personally work with you throughout this program to make certain that you are learning all the basics needed to succeed. Together we will make sure you reach your goals.

Additional programs for intermediate and advanced students have been developed so that by the time you finish this starter plan, you can progress to the next module, and continue your journey toward achieving and maintaining ideal health. I am so excited to get started… I hope that you are too.
